Title V of the Gramm-Leach Bliley Act (GLBA) generally prohibits any financial institution, directly or through its affiliates, from sharing non-public personal financial information about you with a non-affiliated third party unless the institution provides you with a notice of its privacy policies and practices, such as the type of information that it collects about you and the categories of persons or entities to whom it may be disclosed.

Both independent insurance agents and insurance carriers are considered financial institutions under the GLBA. In compliance with the GLBA, we are providing you with this information, which notifies you of the privacy policies and practices of Combined Agents of America, LLC.

We collect non-public personal financial information about you from the following sources: Information we receive from you on insurance applications or other forms; Information about your transactions with us, our affiliates, our insurance carriers or others; Information we receive from a consumer reporting agency, your claims history and other third parties, which we use to perform a service and provide a product that you have requested

We may disclose the following kinds of non-public personal financial information about you.

Information we receive from you on insurance applications or other forms, such as your name, address, social security number, assets, income, drivers license number; Information about your transactions with us, our affiliates or others, such as your policy coverage, premiums, payment history and parties to transactions; Information about you to non-affiliated third parties as permitted by law.

Non-affiliated third parties to whom disclosures may be made:
We may disclose non-public personal information about you to the following types of third parties, unless you tell us not to:

Financial service providers, such as mortgage companies, banks, securities broker-dealers, insurance carriers and insurance agents; non-financial companies, such as title companies, retailers, wholesalers, and insurance surplus lines agencies or brokers; information about you to other non-affiliated third parties as permitted by law

If you prefer that we not disclose non-public personal information about you to non-affiliated third parties and with to opt out, you may opt out of these disclosures, that is, you may direct us not to make those disclosures (other than those disclosures that are expressly permitted by the Gramm-Leach Bliley Act and its implementing regulations) by:

Information we share for Joint Marketing Purposes:
We may disclose all of the information we collect as described above to companies that perform marketing services on our behalf or to other financial institutions with whom we have joint marketing agreements.

Our practices regarding information confidentiality and security:
We believe it is vitally important that the information we maintain about you is as accurate as possible. We maintain internal controls and procedures to ensure the accuracy of your information. Should you have any questions or concerns about the accuracy or completeness of your information, please contact us and we will take immediate steps to correct the information or amend our records.
